Fundraising Event Notification Form

Because of specific IRS tax disclosure and substantiation requirements, KU Endowment must track financial information related to fundraising events. We need your help to ensure that we have the required information.

We must document a good faith estimate of the fair market value of goods and/or services received for contributions made for fundraising events; and report certain information on IRS Form 990. This form and the Fundraising Event Wrap-Up Form will help you to provide this information to us.

Examples of methods used to a make a good faith estimate of the fair market value include, but are not limted to, the following: 1.) Using estimated costs that will be incurred to provide the goods or services. Please note that the costs that would have been charged for items that are donated by a vendor, such as a caterer donating food and wait staff, should be included even if no cash is being expended for these items. Please see the KU Endowment Fund Administration & Use Handbook, Section 4.1 for more details. 2.) Documentation from the internet, or other sources, of the prices charged by for-profit companies for comparable goods and/or services that are sold to the public.
